Беременность и язвенная болезнь.

Продолжаем тему о патологии органов пищеварения из числа экстрагенитальных заболеваний.
Чтобы иметь здоровое потомство, нужно оздоровление всех очагов заболеваний.
Язвенная болезнь – это хроническое заболевание. Имеет свои особенности:
– встречается чаще, чем другие заболевания желудочно-кишечного тракта)
– хроническое течение
– частые рецидивы (частые обострения, сменяющиеся с ремиссией)
– сезонность обострения (весеннее-осенние периоды)
– возникновение язвы в желудке или двенадцатиперстной кишке)
– женщины заболевают реже, чем мужчины (и все же заболевают).
Имеют место основные и предрасполагающие факторы для развития язвенной болезни.
Основные факторы, способствующие возникновению язвы:
– нарушение нейро-гуморальных механизмов, регулирующих процессы пищеварения
* нервно-психические факторы
* гормональные нарушения (нарушение функции гипофиза и надпочечников)
– нарушение местных механизмов пищеварения
– изменение структуры слизистой оболочки желудка
– изменение структуры слизистой оболочки двенадцатиперстной кишки.
Предрасполагающие факторы:
– наследственность
– конституциональные особенности
– условия внешней среды
* нарушение режима питания
* воздействие лекарственных препаратов и других химических веществ…
У беременных основная симптоматика язвенной болезни такая же, как и вне беременности:
– Боли – разнообразного характера, обусловлены
1. повышенным тонусом двенадцатиперстной кишки и желудка
2. увеличением давления в двенадцатиперстной кишке и желудке
3. спазмы в области двенадцатиперстно-желудочного отдела;
Очень редко язвенная болезнь протекает безболезненно – «немая язва». Для нее характерны (из-за кровотечения):
* «кровавая рвота»
* дегтеобразный стул.
– Тошнота, рвота, отрыжка, изжога, запоры. Изжога может появиться и при пониженной кислотности, и даже при полном отсутствии соляной кислоты в желудочном содержимом. Причина изжоги чаще всего, – из-за забрасывания желудочного содержимого в пищевод (в желудочном содержимом есть кислые продукты, образующиеся в результате брожения пищевой массы).
Беременность благоприятно влияет на течение язвенной болезни – во время беременности сохраняется ремиссия язвенной болезни. Благоприятное течение язвенной болезни во время беременности, возможно, связано с рядом факторов, возникающих в результате беременности:
– изменение секреторной функции желудка:
* снижение кислотообразования
* повышение слизеобразования
– понижение моторной функции желудка
– усиление кровоснабжения желудка
– снижение уровня гормона гастрина (при поступлении пищи в желудок – вырабатывается гормон гастрин, стимулирующий выработку соляной кислоты).
Появление острой язвы впервые во время беременности – явление очень редкое. Осложнения (кровотечение, перфорация язвы, стеноз) язвенной болезни во время беременности тоже встречается крайне редко.
Неосложненная язвенная болезнь не является противопоказанием для наступления и продолжения беременности и родов.
При осложненной язвенной болезни беременность отягощается всеми теми последствиями, как и при любой другой тяжелой экстрагенитальной патологии.
Многие симптомы осложнений беременности похожи на симптоматику язвенной болезни (их может дифференцировать врач). Поэтому очень важно раннее обращение к врачу при любом отклонении в состоянии беременной женщины.
Беременность также допустима при отсутствии части желудка после ее резекции по поводу язвенной болезни, под тщательным наблюдением врачей.
